I was once believed to be an innocent benign disease😇
I rarely went on to become a chronic liver disease.
Sometimes, I also turn into liver cancer 💣
But, to my defence, I was created by harmless build up of fat 🥺
Now, I have turned into a silent killer!😈
People call me.......FATTY LIVER
NAFLD or Fatty liver has been claiming lives worldwide and the number is increasing every year.
By 2030, NASH will be the leading cause of cirrhosis and liver failure.
We need to catch this silent killer before it causes more havoc.
It is now time to unsilence it.
Before we see how, let us first see what exactly is NAFLD and NASH.
Non - Alcoholic Fatty Liver Disease (NAFLD) is the presence of steatosis without hepatocyte injury.
Non - Alcoholic Steatohepatitis (NASH) is the presence of steatosis with inflammation and hepatocyte injury, which may be with or without fibrosis.
So how exactly does the innocent Fatty Liver turn into a killer? Let us track the natural course of fatty liver?🕵️♀️
NAFLD turns into NASH, which further develops into Cirrhosis which could either lead to HCC or could potentially lead to Liver failure or death.
Globally, NAFLD has been reported to be more prevalent in individuals with the following risk factors:
- Hyperlipidemia
- Obesity
- HTN
- T2DM
There has also been emerging association of NAFLD with PCOS, Hypothyroidism, OSA, Hypopituitarism, Hypogonadism and surgeries like pancreato - duodenal resection.
Couple of theories like Insulin resistance and Two - hit hypothesis have been suggested as a cause of the devastating effects of NAFLD, however the pathogenesis is not clearly understood🤷🏻
What is worse, fatty liver can also cause extra-hepatic disorders like CVD, CKD and evidence has also shown its association with development of colorectal cancer.
Then why is this killer known to be 'silent'?🧐
Because the LFTs are normal in 50% of the patients with NAFLD, and in 20% of patients with NASH😓
So how do we evaluate these patients and how do we prevent fateful events like HCC and Liver failure❓
