One Pill, Better Cure: Oral Fluconazole Beats Topical Therapies for Vulvovaginal Candidiasis

28 Mar 2025 • A meta-analysis of 50 randomized trials evaluated the efficacy of single-versus multiple-day topical and oral treatments for uncomplicated vulvovaginal candidiasis (VVC).

Key Findings

  • A single 150mg dose of oral fluconazole was more effective than multiple-day topical therapy for early clinical cure (OR 1.52).
  • For late mycological cure, single-dose fluconazole also outperformed topical treatments (OR 1.42)
  • In the ranking analysis, single-day oral fluconazole had the highest probability of achieving both early (SUCRA = 74.1%) and late (SUCRA = 84.2%) mycological cure.
  • Itraconazole, however, ranked highest for both early (SUCRA = 99%) and late (SUCRA = 75.7%) clinical symptom relief. These results suggest that oral azoles offer greater clinical and microbiological success, making them the preferred option for VVC treatment.
