New CHEST Guidelines Warns Against Fresh Frozen Plasma and Platelet Transfusions in Critically Ill Patients

5 May 2025 • A recent guideline from the American College of Chest Physicians (CHEST) advised against routine transfusion of platelets and fresh frozen plasma (FFP) in critically ill adults.

Transfusions are not recommended in stable non-bleeding patients with thrombocytopenia or coagulopathy and are advised only when platelet counts fall below 10 × 10⁹/L (without spontaneous bleeding), 30-50 × 10⁹/L (with spontaneous bleeding risk), or <50 × 10⁹/L (in actively bleeding patients).

Routine prophylactic transfusion before bedside procedures is discouraged, except in rare cases like lumbar puncture.
