Acute respiratory distress syndrome (ARDS) is noncardiogenic pulmonary edema that manifests as rapidly progressive dyspnea, tachypnea, and hypoxemia. Diagnostic criteria include onset within one week of a known insult or new or worsening respiratory symptoms, profound hypoxemia, bilateral pulmonary opacities on radiography, and inability to explain respiratory failure by cardiac failure or fluid overload. Mechanical ventilation remains the most important aspect of managing patients with ARDS. An in-depth knowledge of lung protective ventilation, optimal PEEP strategies, modes of ventilation and recruitment maneuvers are essential. Management of ARDS – What Works and What Does Not

Central line placement is an essential skill, especially in emergency care settings. Indications for central venous line placement include fluid resuscitation, blood transfusion, drug infusion, central venous pressure monitoring, pulmonary artery catheterization, emergency venous access for patients where peripheral access cannot be obtained, and transvenous pacing wire placement. The ideal catheterization site should bear fewer thromboses, lower infectious rate, and fewer mechanical complications. Non-tunneled catheters are commonly used in emergency situations since it is easy to place and does not need special surgical skills for insertion. Complications of CVL placements include arterial puncture, catheter malposition, pneumothorax, subcutaneous hematoma, hemothorax, and cardiac arrest (exceedingly rare). Food hypersensitivity is defined as an adverse reaction to food or a food additive and can be mediated by two different mechanisms: immunologic and non-immunologic. Immunologic reactions are divided into 3 groups: IgE-mediated (allergic reactions), non-IgE mediated (cell-mediated) or both. The spectrum of clinical manifestations is varied such as ranging from mild manifestations (i.e. flushing or rhinorrhea) to life-threatening situations (i.e. anaphylaxis), or can be the cause of worsening pre-existent diseases, such as atopic dermatitis. The majority of patients with a pneumothorax first present to the emergency department and hence the physician needs to be up to date about its presentation and treatment. Pneumothorax should be suspected in patients who present with acute dyspnea and chest pain (classically pleuritic), particularly in those with an underlying risk factor. The treatment focuses on five key areas: air elimination, minimising air leakage, pleural fistula healing, facilitating re-expand, and preventing subsequent episodes. Patients with primary spontaneous pneumothorax (PSP), one that presents in the absence of an external factor, should receive resuscitation with a focus on airway stabilization as well as supplemental oxygen which treats hypoxemia.
