• This study reviewed 73 studies with 92,881 offspring of women with and without PCOS to analyze the impact on birth outcomes.

• Mothers with PCOS were found to be younger with higher BMI and weight gain during pregnancy. The odds of preterm birth, fetal growth restriction, and low birth weight were higher, with lower mean birthweights and higher small for gestational age.

• Identifying PCOS during pregnancy can help improve birth outcomes.
