So what does the evidence tell us?π§
Before we look at the data, let us go back in time to the 60s, when bypass surgeries started gaining popularity.π°
Pretty much anybody with a coronary artery block, single vessel or multivessel, was getting a CABG done.
Let us fast forward to 1971, when PCI was introduced in Zurich and took the world of cardiology by storm.
As PCIs started being well - received, both by the healthcare industry and CAD patients, there were also negative reports of restenosis of the stents in many patients.πββοΈ
So then came a game changer in interventional cardiology - Drug Eluting Stents.πββοΈ
And the craze began yet again over stents in management of coronary artery diseases, so much that there was a case report published in JACC:
'A Heart with 67 stents': This individual had 67 stents done in 28 cath lab procedures in a period of 10 years.π±
Hence both interventional cardiologists and CT surgeons came together to create guidelines on using PCI and CABG.π§ββοΈ
This step was followed by various trials like the SYNTAX trial and the FREEDOM trial, in which cohorts favoured the use of CABG over PCI, with a coronary artery bypass graft showing significantly lower rates of death and MI.π
The general consensus was that CABG can bypass the proximal 2/3 of the vessel, where current and future threatening lesions occur and can be used for complicated lesions, however, stenting has a shorter recovery time.
The verdict is that both these forms of treatment will continue to evolve and has to be chosen on a case-by-case basis.β
