Transfusion Strategies in Acute Myocardial Infarction & Anemia: Liberal vs. Restrictive Approach

28 Dec 2023 • Indications for red-cell transfusion has remained controversial in patients with acute myocardial infarction and anemia patients, given the paucity of evidence.

As per study findings published in NEJM, in patients with acute MI and Hb levels below 10 g/dL, a liberal transfusion strategy (cutoff <10 g/dL) failed to offer significant clinical benefits over a restrictive transfusion strategy (cutoff at 7 or 8 g/dL).

While the liberal approach resulted in a higher mean number of transfused red-cell units, it did not significantly reduce the risk of recurrent MI or death at 30 days in comparison. (14.5% vs. 16.9%).
